Good itinerary for 2-3 week stay in Japan

I am thinking about visiting Japan with my dad this summer (sometime between June-July). I have visited Japan a few times when I was younger but I have not been back since 2004.

In the past I have visited Osaka, Tokyo, Kobe, Kyoto and Hokkaido. I wouldn't mind going back to these places but I don't know if people have other recommendations? If so, please tell me some of your highlights and if you could also provide a 2-3 week itinerary and estimated budget, that would be much appreciated. Lastly, if you have any recommendations on how to travel between all these places (ie. by bus or train) and if you have any accommodation suggestions, that would be great. I would be comfortable spending $40-50/night per person on accommodation. Normally I wouldn't mind staying in a youth hostel but as I will be travelling with my dad, I'm not sure if that would be the best option. Alternatively, we could rent a private room in a hostel or maybe go the airbnb route.

I am interested in pretty much everything - the outdoors, arts & culture and food.
Thanks very much!

Will you be flying into/out of Tokyo, Kansai, or somewhere else? Honshu offers the most flight options so, assuming you'll be in that part of the country, you might consider Hiroshima, Miyajima, and Nara to see some interesting locales without duplicating what you've already seen. Kurashiki is nice too. Of course, as you say, you could certainly return to places like Tokyo and Kyoto because there's always more to see.

If you can, consider flying in and out of Fukuoka and spend your time exploring Kyushu. It's beautiful! You can get around wel by train. In fact, there's now a Shinkansen for Kyushu.

yes, I was going to come in to suggest Kyushu as well.
If you prefer to stay on Honshu you could do a bit of an alternative route starting in Tokyo (if that's where you are flying into) through central Honshu with visits to Matsumoto, Takayama, Kanazawa, down past Biwa-ko to the Kansai area (Kyoto, Nara, maybe into Wakayama to visit Koya-san etc. Kurashiki is nice, you could cross over to Shikoku as well. Ah.... so many options!

I've chosen to go to Tohoku (northern Honshu) next month as an alternative to the big tourist areas for some time travelling around which I'm very excited about to explore.

Yes, Matsumoto! It's in the middle of the Japanese Alps, which are beautiful but very often missed out on a traveller's itinerary. June/July is a great time to go there, as all the alpine areas are open with great hikes, lakes, scenery, etc.

Matsumoto has the kind of budget accomodation you're looking for, plus it's a great base to see the surrounding area (and is a cool city in itself). From there you could go to Norikura Kogen (one of the most beautiful places in Japan, at least without going all the way to Hokkaido) or hike the 6 miles of the Nakasendo (the ancient postal trail) between Tsumago and Magome (both of which still retain their Edo-period character). Matsumoto's also famous for its many onsen and one of the coolest castles in Japan (called "Crow castle" because of its menancing black appearance staring out over the water). It's one of only 2 that haven't been rebuilt. The other authentic one is Himeji.

Or have you thought about climbing Mount Fuji? It's really high but much easier than you might think. It's open in July & August.

Kyushu would be great too. Nagasaki is very cool/different.
